Who were you banned by: I have no idea. I think it was automatic.

Why were you banned: I was banned for accidentally teamkilling one of the guys in my team when I was trying to kill The Hidden that was attacking him. That was the second time that happened to me so I was banned. The first time this happened to me The Hidden was attacking one of my teammates and when I went to shoot The Hidden from behind he jumped and I killed my teammate instead. He forgave me though.

What server where you banned from: The Hidden Source servers.

What date and time were you banned: I was banned yesterday, the 21st of December. I don't remember what time I was banned.

Additional comments: I believe this ban was unfair and I'd really like to play on your servers again because I have had the most fun there then on any other servers. Also, I believe such a ban should only be dealt if you've teamkilled 3-5 people in one map because accidents like these do happen when you're using the shotgun ^^

Neither of these kinds of bans happen without good reason. Care to try and explain them for me?
You've also been banned on two other servers/banlists that have been taken down.

Send me your hidden folder with all of it's files. I'm going to try and look for a reasonable explanation that you had your mat_wirerame set to 2 instead of the server allowed 0.
